Preventous Collaborative Health is looking for an experienced LPN with strong nursing knowledge, excellent assessment skills, and sound clinical judgment to join our collaborative healthcare team.

This is not simply a task-based nursing role. We are looking for a confident nurse who can assess patients, recognize clinical concerns, think critically, and work closely with physicians and nurse practitioners to provide proactive, preventative care.

What You'll Do

- Complete patient assessments and review health histories
- Identify abnormal findings or clinical concerns and escalate appropriately
- Prepare patients for physician and NP appointments
- Perform nursing procedures, testing, treatments, injections and immunizations within LPN scope
- Support preventative screening and chronic disease management
- Provide patient education and coordinate follow-up care
- Maintain accurate clinical documentation
- Work collaboratively with our multidisciplinary healthcare team

What We're Looking For

- Current LPN registration and good standing
- 2+ years of nursing experience preferred
- Solid nursing knowledge and assessment skills
- Excellent clinical judgment and critical thinking
- Confidence working independently and recognizing when concerns require escalation
- Strong communication,



organization and follow-through
- Primary care, family medicine, chronic disease, preventative health or urgent care experience is an asset
- Experience with ECGs, spirometry, injections, bladder scanning, specimen collection and EMRs/CHR is an asset
