Community nursing at ParaMed combines patient assessment and care planning with scheduled visits throughout County Area, Erie St. Clair District. Support patients across ages and care complexities in their homes.

As a Registered Nurse, you will provide care in various settings while helping patients remain comfortable at home. You will assess individualized needs, execute care plans, and monitor outcomes continuously. Building rapport and trust supports collaborative patient engagement and therapeutic relationships.

Key Responsibilities:

- Assess individualized patient needs using appropriate tools and techniques
- Execute and continuously monitor individualized care plans
- Build rapport and trust through collaborative patient engagement
- Maintain and update professional patient encounter records
- Document plans related to patient care

Requirements:

- Current CNO registration as a Registered Nurse in valuable standing
- Valid driver’s license and access to a vehicle
- Community care experience considered an asset
- Reliability, flexibility, and willingness to travel
- Required after-hours on-call coverage for patient safety
