Work directly with clients, families, and care providers. This Community Mental Health Case Manager supports individuals aged 16 and older who require assistance living and working in the community. The work includes intake assessments, client advocacy, and coordination across mental health, criminal justice, developmental, and addiction services. Provide one-to-one case management for community-based clients Complete intake assessments and manage client crises Advocate for clients and collaborate with care providers Participate in supervision, meetings, events, and agency functions Minimum three years of community case management experience Degree or diploma in social services, or equivalent experience Awareness of trauma's impacts on mental health Understanding of mental health systems and consumer perspectives Bring trauma awareness, advocacy, and coordinated case management to clients pursuing stability, independence, and improved quality of life.
