Key Responsibilities
Review and verify prescriptions before medication distribution Complete medication reconciliation, consultation, counselling, and follow-up Develop and monitor medication care plans Contribute to interdisciplinary care rounds Advise teams on drug therapy and therapeutic outcomes

Requirements:
Pharmacy degree from an accredited post-secondary program Registration and good standing with Ontario College of Pharmacists Ability to perform technical medication dispensing duties Critical thinking skills for independent client care Commitment to client-centred care and health teaching Apply clinical pharmacy, medication counselling, and cooperative care skills across VHA's home and community services.
