Fraser Health Authority is hiring Casual Social Workers for Long Term Care in Surrey, BC. You will conduct psycho-social assessments, provide clinical counseling, and support families and residents facing chronic illness and transition challenges.

The role collaborates with an interdisciplinary team to plan care and connect clients to community resources. Reporting to the Qualified Practice Leader, you’ll contribute to care planning, participate in in services, and help maintain documentation
