Registered Social Workers provide psychosocial assessment, counselling, and discharge planning across St. Joseph’s Health System sites in Hamilton. This is regular full-time daytime work.

The Social Work Services department seeks one Registered Social Worker for hospital-based practice. The position supports patients and families through case management, service coordination, patient education, and consultation. It also addresses high-risk social situations while supporting recovery, quality of life, and timely discharges.

Key Responsibilities:

- Conduct comprehensive psychosocial assessments from a systems perspective
- Provide counselling, crisis intervention, and illness adjustment support
- Coordinate complex discharges and case management activities




- Facilitate family meetings using mediation and negotiation skills
- Document interventions according to professional standards

Requirements:

- Bachelor of Social Work from an accredited university
- Registration with the Ontario College of Social Workers
- Two years of acute care hospital experience preferred
- Excellent interpersonal, communication, organizational, and time management skills
- Knowledge of senior and family care needs
