CarePartners seeks Registered Nurses for home care and community settings. You will develop care plans, deliver in-person and virtual nursing services, and communicate with patients, families, and interdisciplinary teams. The position includes diverse assignments across Owen Sound and surrounding areas.

Key Responsibilities:

- Develop and maintain effective community health care plans
- Provide exceptional nursing services in person and virtually
- Deliver care in homes, clinics, retirement homes, or schools
- Liaise with patients, families, and interdisciplinary care teams
- Perform other clinical duties as required

Requirements:

- Registered Nurse degree or diploma
- Current College of Nurses of Ontario registration
- Valid driver’s license and vehicle for regular travel
- Excellent English verbal and written communication skills
- CPR certification and current immunization record
