AgeCare seeks an experienced Licensed Practical Nurse reporting to the Director of Care. You will support seniors through accurate care plans, documentation, resident care, and treatment administration. Key Responsibilities:

- Apply person-centered care within the inter-skilled team
- Complete accurate resident assessments and care plans
- Evaluate care plans and communicate required changes
- Coordinate care conferences and participate where applicable

Requirements:

- Practical Nurse Diploma
- Current provincial registration in good standing
- Three or more years’ seniors’ health experience
- Current CPR training
