Key Responsibilities:

• Complete comprehensive assessments for individual clients
• Develop care plans based on unique client needs
• Implement and evaluate evidence-based care
• Lead care as the primary provider when designated
• Connect clients with community support services

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario
• Availability for days, evenings, nights, and alternating weekends
• Current CPR certificate
• Updated immunization and required background checks
• Willingness to attain required SE Health certifications
