INTERNAL/EXTERNAL POSTING: ROAD TO RECOVERY (R2R) HOME CASE MANAGER Permanent Base office: Purpose of the Position: The R2R Case Manager is part of a multidisciplinary team and is responsible for providing support to residents within a semi-independent congregate living environment. The Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness and/or addictions; specifically supporting residents of the R2R Home. As a member of a multidisciplinary team and with strong clinical-case management abilities, the member will provide recovery-oriented supports within the congregate living setting. Awareness and application of psychosocial rehabilitation and recovery-oriented services is an asset. Bachelor's degree in a human service/health science discipline relevant to the position. oMental Health related diploma with relevant work experience PLUS registration with a regulated health professional college One year of experience working with persons who have mental illness and/or developmental disability and/or substance use issues preferred, plus experience in providing one or more of the following; o supportive counselling o case management o crisis intervention Manage internal and external relationships, to have and demonstrate empathy and compassion, to be aware of emotional intelligence, conflict resolution, and the understanding of the feelings, ideas and beliefs of others. Indigenous cultural awareness and education is an asset. Bilingual in French is an asset. Ability to communicate effectively, both orally and in writing. Tolerance of and ability to handle a heavy workload and competing demands and high risk situations. Ability to use and learn new technology (computers, software programs, phones, printer/scanner, alarm/security systems etc.). Willingness to work days/evenings/overnight telephone support (including weekends & statutory holidays) 29.85 to $39.09 per hour commensurate with qualifications and experience
