Provide patient-centred nursing care as a Part Time Registered Practical Nurse at Providence Transitional Care Centre - Level 3. Practice safely under full scope within an inter-professional team.
At Providence Care, the Registered Practical Nurse delivers effective care in a non-acute setting. You will support patients with established complexity of care needs and predictable outcomes. The position addresses physical, emotional, social, and spiritual needs while helping patients and caregivers manage clinical conditions and transitions.
Key Responsibilities: Deliver protected, effective, patient-centred care
Evaluate patients with established complexity of care needs
Establish preventive, therapeutic, and rehabilitative care priorities




Advocate for patients and their caregivers
Evaluate and modify care plans using professional judgment
Requirements: Active unrestricted College of Nurses of Ontario license
Current CPR certificate
College certificate required; diploma preferred
Post-1985 medication certificate when applicable
Ability to seek assistance beyond RPN scope
Bring qualified judgment, patient advocacy, and safe nursing practice to Providence Care’s transitional care patients and caregivers.
