Summary
As an Addictions & Wellness Team Lead, you will support coordinated care for people navigating addiction and wellness needs. Your impact: Clear case coordination can help clients and care partners understand next steps and access more consistent support.
Responsibilities
- Coordinate case management activities to support continuity of care.
- Guide team members in delivering patient-centred support.
- Communicate relevant care information with clients and appropriate care partners.
- Use medical terminology accurately when documenting or discussing care.
- Support a respectful, wellness-focused environment for people seeking help.
- Qualifications
- Required:
Patient care skills relevant to supporting people with addiction and wellness needs.
- Required: Case management skills to coordinate care and follow-up.
- Preferred: Familiarity with medical terminology.
- Experience working with children, hospital experience, pediatrics, research, data collection and special education were supplied but are not established as requirements for this role.
