Job Summary

We are seeking an experienced RN or RPN to provide home nursing care for an adult with complex care needs, working collaboratively with a PSW and the clinical care coordinator.

Requirements

- Current registration in good standing with the College of Nurses of Ontario.
- Current Basic Life Support certification.
- Recent hands-on experience providing independent community nursing or comparable complex care.
- Demonstrated competency in gastrojejunostomy tube care, enteral feeding pumps, prescribed flushing and medication administration through feeding tubes.
- Demonstrated competency in PICC line care, aseptic technique, prescribed intravenous medication administration and infusion pumps.
- Experience administering subcutaneous medications and managing subcutaneous butterfly devices.
- Experience assessing pain and monitoring patients receiving opioids and other medications that may affect alertness or respiratory status.
- Ability to recognize and respond to feeding intolerance, device complications, infection, sedation and changes in clinical condition.
- Strong medication reconciliation, calculation, documentation and clinical assessment skills.
- Experience assisting wheelchair users and following an assessed pivot-transfer plan.
- Ability to work collaboratively with PSWs,



family caregivers and existing nursing providers while maintaining transparent care responsibilities.
- Availability for consistent eight-hour daytime shifts; schedule and start date to be confirmed.
- Reliable transportation to the Goderich assignment.
- Comfortable working in a home with dogs.
- Completion of client-specific orientation and competency verification before independent assignment.

Qualifications

- Preferred experience with chronic pain management, gastroparesis, reduced mobility and caregiver education.
- In good standing as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO.
- Relevant nursing experience in home care and patient care; experience in hospital or acute care settings is an asset.
- Experience or familiarity with case management, pain management, medication administration, and phlebotomy is an asset.
- Experience working with older adults, or senior care or dementia care is valued.
- Background in assisted living, nursing homes, medical offices, or is an asset.
- Knowledge of medical terminology, strong clinical judgment, and clear documentation and communication skills.
- Ability to work independently in community settings while collaborating effectively with clients, families, and the care team.

Pay: $45.00-$55.00 per hour
