Care Manager (Registered Nurse) – Primaris Health (Calgary, AB)

About Us

Primaris Health is a private, membership-based clinic in Calgary offering proactive, team-based care. Our physicians, nurses, mental health providers, naturopathic doctors, dietitians and physiotherapists work together to give every patient coordinated, personal care.

The Role

As our Registered Nurse Care Manager, you'll be the main point of contact between our patients and care team. You'll coordinate and monitor each patient's care, explain treatment plans in plain language, and lead day-to-day nursing coordination with our LPNs.

What You'll Do

- Coordinate referrals, diagnostics, specialist visits and follow-ups
- Assess patients' clinical, social and functional needs using screening tools
- Build and update individualized care plans
- Assign daily patient workloads to LPNs and review results with physicians
- Educate patients and families, and advocate for them across the healthcare system
- Identify patients due for screenings and support those with complex chronic conditions
- Keep accurate EMR records in line with Alberta's Health Information Act




- Take part in interdisciplinary care rounds and help improve clinic workflows
- Collaborate with the whole team to provide care management for the patient

What You Bring

- Registered Nurse in good standing with CRNA (or eligible)
- 3+ years of nursing experience, ideally in primary care or chronic disease management
- Experience with care planning and coordinating across providers
- Comfortable with EMRs; current BLS/CPR
- Warm, explicit communicator who is highly organized
- Nice to have: case management experience, preventive or concierge care background

Pay: $60.00-$70.00 per hour
