Our mission is to address medical and social determinants of health, with a strong emphasis on equitable access for diverse and equity-deserving populations, including vulnerable groups. We deliver care through in-person, virtual, and mobile services.

About the Role
We are seeking a Nurse Practitioner to join a growing Interprofessional Primary Care Team (IPCT) that is evolving beyond a traditional primary-care clinic toward a more integrated, population-health and complex care model. The NP will balance primary care with consultative and shared care support for patients referred from community primary care providers who would benefit from additional interprofessional assessment, stabilization, care planning and follow-up.
Key Responsibilities
· Provide comprehensive primary care, including assessment, diagnosis, treatment, health promotion, disease prevention and chronic disease management.
· Manage a defined primary-care population while also supporting externally referred patients with complex, high-risk or rising-risk needs.
· Complete comprehensive reviews of complex patients,
including medication optimization, diagnostic and laboratory review, referrals, specialist recommendations, care gaps and current care plans.
· Identify early signs of deterioration and intervene proactively to reduce avoidable emergency department use, hospitalization and fragmentation of care.
· Develop and update shared care plans in collaboration with RN and Social Work Care Managers, RPN Coordinators, Pharmacists, Physicians, community partners and the patient/family.
· Provide short-term consultative or episodic support to community primary-care providers while maintaining explicit communication and recommendations.
· Coordinate transitions of care and ensure timely follow-up after hospital, specialist or community-service encounters.
· Use virtual, in person and mobile models of care as appropriate to improve access for vulnerable and equity-deserving populations.
· Contribute to team huddles, complex-care case review, quality improvement, population-health initiatives and development of standardized clinical workflows.
