Coordinate resident personal care services at AgeCare Willowgrove using PSW leadership, care audits, and person-centered support. Work with the Director of Care and senior management team.

The PSW Coordinator plans, organizes, directs, and coordinates services within approved policies and procedures. This position supports PSWs through hiring, orientation, evaluation, coaching, mentoring, and education. It also promotes AgeCare's mission of prioritizing resident health, well-being, and comfortable home-like care.

Key Responsibilities:

- Manage daily Personal Support Worker operations with the Director of Care
- Participate in PSW hiring, orientation, evaluation, coaching, and mentoring
- Coordinate onboarding documentation, care plans, reports, and orientation shifts
- Monitor resident care through supervision, leadership rounds,



and formal audits
- Analyze audit results and report findings at monthly quality meetings

Requirements:

- Graduate of a qualifying Personal Support Worker certificate or diploma program
- Personal Support Worker experience in long-term care
- Knowledge of straightforward treatments and related bedside care procedures
- Understanding of approved policies and procedures for resident services
- Ability to support PSWs through coaching, mentoring, and education

Support AgeCare Willowgrove residents through PSW coordination, quality auditing, mentorship, and attentive personal care service leadership.
