Deliver complex pediatric home care with VHA Home HealthCare in Toronto Central, using assessment and care coordination skills. This full-time visiting position requires travel.

As a Registered Nurse or Registered Practice Nurse, you will support VHA’s Child and Family program. The position provides individualized, one-on-one care for newborns through young adults up to 25. You will deliver home-based nursing care while supporting health and well-being in family-centered settings.

Key Responsibilities:

- Assess clients and develop care plans
- Coordinate nursing services for children and families
- Evaluate interventions through ongoing reassessment
- Document care according to skilled standards
- Participate in standby and on-call rotation

Requirements:

- Registration in good standing with the College of Nurses of Ontario
- RN or RPN diploma or recognized degree
- Valid First Aid or CPR certification
- Ontario G2 or G driver’s license and reliable vehicle
- Effective communication and documentation skills
