Support community-dwelling seniors through outpatient occupational therapy at Sunnybrook Health Sciences Centre’s Peter Cipriano Centre. Apply geriatric assessment and rehabilitation expertise.

This occupational therapist supports the Geriatric Day Program, Geriatric Medicine and Specialized Clinic, and Geriatric Outreach Services. The role provides rehabilitation for up to eight weeks or comprehensive geriatric assessments. You will assess occupational performance, develop treatment plans, and collaborate with families, caregivers, and interprofessional providers. Practice follows College of Occupational Therapists of Ontario standards.

Key Responsibilities:

- Conduct occupational therapy assessments for safe community living
- Complete comprehensive geriatric assessments
- Develop individual and group-based treatment plans




- Collaborate with clients, families, and caregivers
- Teach students and support evidence-based practice

Requirements:

- Registration with the College of Occupational Therapists of Ontario
- Minimum three years supporting older adults with frailty
- Strong cognitive assessment and documentation skills
- Experience facilitating senior group therapy and fall prevention
- Ability to work within interprofessional teams
