Terms may be affected by collective bargaining. Responsibilities Conduct individual nutrition counselling – assess client status, habits, socio‑economic, cultural, psychological and medical factors. Develop, implement and evaluate individualized nutrition care plans based on client health, literacy, change stage, culture and economic context. Maintain accurate records, monitor clients regularly, and participate in case conferences, team meetings and quality improvement initiatives. Plan, lead, and evaluate health promotion and educational activities, including community outreach and nutrition education sessions. Identify barriers to local food access and facilitate community initiatives to improve access. Collaborate with multidisciplinary teams to develop health promotion programmes; assess needs, plan and evaluate initiatives. Advocate for policy and organisational support for food and nutrition issues. Build and maintain networks with community organisations for team-oriented initiatives. Provide nutritional consultation and capacity building to staff and community partners. Pursue professional development and maintain competence through networking and continuing education. Skills & Qualifications Bachelor’s in Foods and Nutrition or equivalent from a recognised university. Registration with the College of Dietitians of Ontario. Three to five years of clinical counselling and community nutrition experience. Comprehensive knowledge of nutritional care and demonstrated counselling skills with individuals and groups. Effective teamwork in interdisciplinary settings. Excellent interpersonal, communication, decision‑making, problem‑solving, conflict management and time‑management skills. Experience in programme development, implementation, monitoring and evaluation. Proficiency in a second language relevant to the catchment area is an asset. Working Conditions & Physical Requirements Some evening and/or weekend hours will be required based on project demands.
