The Clinical Care Coordinator supports the Director of Care in leading the nursing department and ensuring residents receive safe, high-quality, person-centred care that reflects their needs, preferences, and rights. The role provides leadership, coaching, and supervision to nursing staff; promotes compliance with legislation, policies, clinical documentation, infection control, and safety standards; coordinates admissions, transfers, discharges, supplies, education, quality improvement, and risk management; collaborates with residents, families, multidisciplinary teams, physicians, regulators, and community partners; and provides direct clinical care when required.

The Responsibilities

- Provide effective leadership, coaching, and supervision to nursing staff.
- Ensure compliance with legislation, organizational standards, policies, and procedures.
- Delegate duties appropriately to regulated and non-regulated nursing staff.
- Maintain effective communication within nursing and across other departments.
- Ensure nursing leadership and management coverage, including on-call responsibilities.
- Manage nursing resources, budgets, equipment, and supply inventories.
- Maintain a secure environment and ensure adherence to infection prevention and control practices.
- Coordinate resident admissions, transfers, discharges, and multidisciplinary care.
- Ensure high-quality resident care through assessments, accurate clinical documentation, and staff supervision.
- Support continuous quality improvement, risk management,



staff education, and regulatory relationships.

Qualifications

Education

- A Bachelor of Science degree in Nursing OR a Diploma in Nursing, as well as a certificate of competence as a Registered Nurse or Registered Practical Nurse with the College of Nurses of Ontario.

Experience

- At least one year of experience working as a registered nurse or registered practical nurse in the long term care sector is an asset
- At least three years of experience working as a registered nurse or registered practical nurse in a managerial or supervisory capacity in a healthcare setting.

Knowledge

- A sound knowledge and background in geriatric, rehabilitation or long term care nursing and personnel management.
- Knowledge of the requirements of the Fixing Long Term Care Act and corresponding regulations and other pertinent Federal, Provincial and Municipal statutes which govern or influence the operation of the home.
- Knowledge of the nursing process and trends in the long term care field and demonstrated skills in the planning and coordination of a comprehensive nursing program.
