Overview
We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will possess solid organizational and communication skills, with the ability to analyze complex situations and provide effective solutions. As a Case Worker, you will play a vital role in supporting individuals and families in navigating various challenges, ensuring they receive the necessary resources and assistance.

Responsibilities

- Conduct assessments to understand clients' needs and develop tailored service plans.
- Communicate effectively with clients, providing guidance and support throughout their case management journey.
- Write detailed reports and documentation to track client progress and outcomes.
- Organize case files and maintain accurate records using SharePoint and Office applications.
- Analyze client data to identify trends and areas for improvement in service delivery.
- Present findings and recommendations to team members and stakeholders.
- Collaborate with community resources, agencies, and service providers to facilitate client access to necessary services.
- Maintain confidentiality of all client information in accordance with ethical standards.

Requirements





- Proven experience in case management or a related field is preferred.
- Strong organizational skills with the ability to manage multiple cases simultaneously.
- Excellent written and verbal communication skills, including the ability to present information clearly.
- Proficiency in Microsoft Office Suite, particularly Outlook for communication purposes.
- Familiarity with SharePoint for document management is a plus.
- Ability to analyze data effectively to inform decision-making processes.
- A compassionate approach towards clients, demonstrating empathy and understanding of their circumstances.
- Fluency in additional languages, such as French, is advantageous but not required.

Job Type: Full-time

Pay: $16.40-$18.50 per hour
