Overview

We are seeking a compassionate and community-driven Case Manager to lead the delivery of a culturally grounded Food Security Program for Indigenous seniors (55+).

Key Responsibilities

Develop and implement individualized support plans for clients

- Build trusting relationships with Indigenous seniors through outreach, intake, and ongoing support.
- Provide one-on-one case management, including check-ins and referrals.
- Support seniors with grocery planning, budgeting, and access to food resources.
- Coordinate and facilitate small-scale cooking sessions, meal preparation, and food-sharing activities.
- Deliver or coordinate meal support for seniors facing mobility or access barriers
- Integrate cultural and intergenerational elements into programming (e.g., Elder teachings, youth participation)
- Maintain accurate client records and contribute to basic data collection and reporting.
- Work collaboratively with community partners and internal agency team members.

Qualifications

Proven experience in case management or related roles.

- Experience working with First Nations, Metis and Inuit communities is preferred.




- Knowledge of trauma-informed, culturally protected, and strengths-based approaches.
- Background in social work, community development, social sciences, or related field (or equivalent lived experience).
- Strong relationship-building and communication skills.
- Experience with case management, outreach, or community programming.
- Ability to work independently and manage time effectively.
- Valid class 5 driver’s license, clean driver abstract, and access to a reliable vehicle is an asset.

Pay: $37,500.00-$40,000.00 per year
