This Community Outreach Worker supports new and existing clients within service plans and as-needed services. The position helps clients connect with external resources, build skills, and achieve goals. Work occurs Monday to Friday, with possible assignments across multiple locations.

Key Responsibilities:

Requirements:

- Minimum two years’ experience working in community settings

- Valid driver’s license and access to a reliable vehicle

- Excellent verbal, written, and computer communication skills
