Reporting to the Director of Care, you will collaborate with an interdisciplinary team and support transitions, medications, assessments, and care planning.

You bring current LPN registration, robust clinical knowledge, and effective communication. CPR/First Aid, infection prevention, and comfort with electronic documentation are expected
