Support seniors through geriatric outpatient rehabilitation using occupational therapy assessments and interprofessional collaboration at Sunnybrook Health Sciences Centre. Work across the new Peter Cipriano Centre for Seniors Health.

This Occupational Therapist serves seniors living in the community through the Geriatric Day Program, Geriatric Medicine and Specialized Clinic, and Geriatric Outreach Services. The work includes comprehensive geriatric assessments, individual and group treatment plans, psychosocial support, and community resources. The setting prioritizes occupational therapy according to presenting needs and care aims.

Key Responsibilities:

- Conduct occupational therapy assessments for safe community living
- Develop individual and group-based therapy plans




- Complete comprehensive geriatric assessments
- Collaborate with clients, families, and caregivers
- Participate in meetings, rounds, teaching, and family conferences

Requirements:

- Registration with the College of Occupational Therapists of Ontario
- Minimum three years supporting older adults with frailty
- Robust cognitive assessment experience
- Experience facilitating senior group therapy and fall prevention classes
- Strong documentation skills and computer literacy
