Support geriatric patients through assessment, care coordination, and short-term follow-up as a Social Worker, Geriatric Navigator in Toronto. This is a full-time temporary position.

Working within the Medicine Program at St. Joseph’s Health Centre, you will collaborate with the Geriatric Medicine consultation team. The position serves geriatric clients and families through crisis intervention, community referrals, and short-term management. Your work supports patients’ highest functioning in the community.

Key Responsibilities:

- Coordinate post-discharge care with discharge planners and medicine teams
- Develop follow-up plans for patients at high readmission risk
- Make community referrals and coordinate health centre services




- Case conference with providers involved in patient care
- Track performance indicators and quality improvement metrics

Requirements:

- Master’s degree in Social Work
- Ontario College social work registration in valuable standing
- Minimum two years’ geriatric population experience
- Current BLS or completion during probation
- Access to a vehicle and strong communication skills
