Provide advanced foot care as a part time Registered Practical Nurse with VHA Home HealthCare across Central and Toronto Central regions. Use nursing assessment and care coordination in visiting community settings.

VHA’s RPN role delivers home health nursing services to diverse clients, families, and groups. This program serves community clinics, long-term care homes, and home-based settings. You will provide prevention, treatment, and ongoing foot care management while following College of Nurses of Ontario standards, VHA policies, and community health nursing practice.

Key Responsibilities:

- Assess clients and develop care plans and coordination
- Provide individualized advanced foot care services




- Reassess interventions and evaluate care results
- Document care according to College standards
- Report changes to supervisors and case coordinators

Requirements:

- RPN diploma from a recognized college
- Good standing with the College of Nurses of Ontario
- Valid Advanced Foot Care Certificate and 100 service hours
- Valid First Aid or CPR certification
- Ontario G2 or G license and reliable vehicle

Use advanced foot care, clinical judgment, and documentation skills to support client comfort, mobility, and safe community care.
