The Personal Support Worker partners with a Care Designer and broader care team to support secure, comfortable aging. Provide personal care, daily living assistance, vital-sign monitoring, mobility support, and accurate documentation. Communicate client changes promptly while respecting preferences, routines, privacy, and personal goals.

Key Responsibilities:

- Provide care according to established plans and clinical guidance
- Support bathing, grooming, dressing, toileting, feeding, and continence care
- Assist mobility, transfers, repositioning, and range-of-motion exercises




- Observe changes and document care accurately and promptly
- Prepare meals, perform light housekeeping, and provide transportation support

Requirements:

- Personal Support Worker or Home Care Aide certification
- Current CPR certification and required negative TB test
- Valid driver's license and access to an insured vehicle
- Ability to work independently and collaboratively
- Empathy, reliability, communication, and confidentiality commitment
