Lead client assessments and care planning in Toronto and across the GTA, using communication and supervision skills in a community-based role.

As a Client Services Supervisor, you will assess client needs, establish care plans, and supervise Home Service Workers. You will coordinate with Service Coordinators, families, Ontario Health atHome, and agency teams. The work includes staff education, documentation, safety management, and ongoing client follow-up.

Key Responsibilities:

- Conduct ongoing in-home client assessments and reassessments
- Develop care plans based on client needs
- Supervise Home Service Workers through scheduled spot checks
- Coordinate communication about changing client needs
- Manage client safety incidents and required follow-up

Requirements:

- Current registration as a regulated healthcare qualified
- Ability to supervise Home Service Workers
- Excellent interpersonal, problem-solving, and communication skills
- Competence with RAI and Gold Care tools
- Valid driver's license and vehicle required
