Lead daily personal support services in Ottawa with clinical operations, staff coaching, and care planning for Carefor clients. This is a 12-month term.

As Supervisor, Personal Support Services, you will oversee PSW service delivery and assist the Manager with clinical operations. You will complete care plans, engage Ontario Health at Home Care Coordinators, and address service concerns. Evidence-based guidelines and informational data support team performance.

Key Responsibilities:

- Oversee and assist clinical PSS program operations
- Coach and mentor PSS staff toward performance objectives
- Complete annual performance reviews for assigned staff
- Implement Carefor policies affecting personal support services
- Participate in after-hours On Call rotation

Requirements:

- Ontario-licensed Registered Practical Nurse in positive standing
- Minimum two years community nursing or personal support experience
- Minimum one year leadership experience preferred
- Understanding of CNO Standards of Practice
- Vehicle and valid Ontario Driver's license required
