This Community Outreach Worker supports recent and existing clients within service plans and as-needed services. The position helps clients connect with external resources, build skills, and achieve goals. Work occurs Monday to Friday, with possible assignments across multiple locations.
Key Responsibilities:
- Provide support and recovery services to new and existing clients
- Build relationships and advocate for clients' needs
- Connect clients with appropriate external resources
- Help clients develop skills and achieve their goals
- Minimum two years' experience working in community settings
- Experience providing outreach to hard-to-reach populations
- Valid driver's license and access to a reliable vehicle
- Knowledge of crisis management and problem-solving
- Excellent verbal, written, and computer communication skills
