What You’ll Do
- Longitudinal primary care
- Chronic disease support
- Follow-up care
- Relationship building with patients
- Care coordination across providers/services What We’re Looking For:
- Licensed NP (minimum 3 years) to support Newfoundland & Labrador + any of: Alberta, Ontario, British Columbia or Quebec
- Experience in primary care
