This Community Disability Support Worker position provides person-centered support within a residential setting. You will promote independence, social inclusion, and well-being while respecting individual needs and preferences. The work includes collaborating with clients, families, and multidisciplinary teams.

Key Responsibilities:

- Provide personal care, hygiene, meals, and mobility assistance
- Develop individualized support plans with clients and teams
- Facilitate social, recreational, and community participation
- Document progress, health status,



and changing needs
- Advocate for rights and community service access

Requirements:

- High School Diploma or equivalent
- Transparent vulnerable sector criminal record screening
- Experience supporting individuals with disabilities
- Effective spoken and written English communication
- Behavior management or medication training preferred
