As Transition Coordinator at Verve Senior Living, you will guide current residents and families through personalized move-in experiences. You will coordinate plans, logistics, documentation, and follow-up while building trusted relationships. Collaboration with Community Relations, Dining Services, Life Enrichment, and Health & Wellness supports successful community integration.
Key Responsibilities: Serve as primary contact during move-in and first 90 days
Develop personalized transition plans for residents and families
Coordinate movers, elevators, parking, furniture, and move-day requirements




Connect families with relocation and community resources
Coordinate department follow-up and resident integration
Requirements: Diploma or degree in a related field
Three to five years supporting older adults, families, or customers
Strong communication, relationship-building, service, and problem-solving skills
Ability to manage competing priorities and coordinate across teams
Microsoft Office and CRM or resident management systems proficiency
