This Pharmacist practices across VHA’s newly established pharmacy and community settings. You will review prescriptions, resolve drug-related problems, and collaborate with patients, caregivers, primary care providers, and healthcare professionals. The work supports safe medication use and client-centred care.
Key Responsibilities: Review and verify prescriptions before medication distribution
Complete medication reconciliation, consultation, counselling, and follow-up
Develop and monitor medication care plans
Contribute to interdisciplinary care rounds
Advise teams on drug therapy and therapeutic outcomes
Requirements: Pharmacy degree from an accredited post-secondary program
Registration and valuable standing with Ontario College of Pharmacists
Ability to perform technical medication dispensing duties
Critical thinking skills for independent client care
Commitment to client-centred care and health teaching
