Deliver palliative patient care as a Registered Practical Nurse at Sunnybrook Health Sciences Centre, using assessment skills within a primary nursing model.
This Registered Practical Nurse provides total care for a group of palliative care patients within scope of practice. The position supports Person Centred Care and Sunnybrook’s Mission and Values. Collaboration with interprofessional team members is central to the work.
Key Responsibilities: Provide total care for assigned palliative care patients
Practice within the primary nursing care delivery model
Support Person Centred Care concepts and principles
Work closely with interprofessional team members
Apply excellent assessment skills during patient care
Requirements: Current registration with the Ontario College of Nurses
Current B.C.L.S. certification
RPN diploma or equivalent qualification
Required hospice palliative care experience
Required palliative pain and symptom management experience
Bring palliative assessment, communication, and person-centred care capabilities to Sunnybrook’s Veterans Centre K1E Palliative Care service.
