This Community Outreach Worker supports recent and existing clients within service plans and as-needed services. The position helps clients connect with external resources, build skills, and achieve goals. Work occurs Monday to Friday, with possible assignments across multiple locations.
Key Responsibilities: Provide support and recovery services to new and existing clients
Build relationships and advocate for clients’ needs
Connect clients with appropriate external resources
Help clients develop skills and achieve their goals
Deliver culturally appropriate, non-judgmental services
Requirements: Minimum two years’ experience working in community settings
Experience providing outreach to hard-to-reach populations
Valid driver’s license and access to a reliable vehicle
Knowledge of crisis management and problem-solving
Excellent verbal, written, and computer communication skills
