Baycrest Hospital seeks a Registered Practical Nurse with complex-care expertise for temporary part time day shifts and alternate weekends.
This Registered Practical Nurse supports older adults in Complex Continuing Care Unit 5W. The position provides post-acute inpatient care for patients with multiple chronic health conditions. It emphasizes client-centred nursing within an interdisciplinary team, including medications, treatments, and individualized care planning.
Key Responsibilities: Plan nursing interventions, medications, and treatments within scope
Deliver and evaluate nursing interventions for complex medical needs
Provide client-centred personal care, activation, recreation, and spiritual support
Collaborate with Registered Nurses on individualized care plans




Revise individualized care plans with the interdisciplinary team
Requirements: Current good-standing registration with the College of Nurses of Ontario
Current Basic Cardiac Life Support Level C and AED certificate
Geriatric complex medical care experience preferred
Psychogeriatric assessment, intervention, or responsive behaviours experience preferred
Excellent interpersonal, communication, and problem-solving skills
Support medically complex older adults at Baycrest Hospital through nursing interventions, collaboration, and individualized quality-of-life care.
