Key Responsibilities: Act as a health care liaison for the relocation project
Work with the Emergency Project Provider and Kingston healthcare partners
Triage community members visiting the nursing station
Coordinate appointments for the onsite Primary Care provider
Conduct wellness checks for identified community members
Requirements: RPN licence in positive standing
Communicate verbally and in writing in English
Familiarity with First Nations culture
Experience with mental health and substance abuse
Current TB Screen and Criminal Record Check
