Humber River Health is seeking a Social Worker to support acute and community care pathways at multiple sites in the Greater Toronto Area. You will perform psychosocial assessments, discharge planning and counseling for patients and families within a quick-paced hospital setting.

The role requires an MSW, current professional membership, strong communication skills and the ability to work independently and in teams across the patient care continuum.
