• Monitor residents, recognize changes in condition,



and promptly report concerns to the supervising RN and providers.
• Administer medications and treatments according to orders, facility procedures, and your scope of practice.
• Participate in developing, implementing, and evaluating individualized care plans.
• Provide direction and support to nursing assistants within your scope of practice.
• Maintain accurate, timely clinical documentation.
• Communicate with residents and families about care, progress, and concerns.
• Promote resident dignity, safety, infection prevention, and quality care. What You Bring
• An active LPN license authorizing practice in Wyoming.
• Current CPR/BLS certification.
• Solid clinical skills, communication, and teamwork.
