Join our dedicated team as a Licensed Practical Nurse focusing on home care. You will assess, plan, and deliver quality patient-centered care in clients' homes, ensuring optimal health outcomes. As a part-time Licensed Practical Nurse, you'll report directly to the Home Care Manager. Your role includes collaborating with the Registered Nurse Case Manager to evaluate and implement care plans for clients. Utilize your robust assessment skills to provide comprehensive care, leveraging critical thinking throughout the nursing process.

Key Responsibilities
• Assess and monitor client health conditions in their homes
• Collaborate with Registered Nurse Case Manager on care plans
• Provide direct nursing care alongside clients and their families




• Evaluate effectiveness of care and make necessary adjustments
• Maintain accurate documentation of care and client progress

Requirements:
• Accredited practical nurse education program completion
• Active registration with College of LPNs and HCAs of Alberta
• Minimum two years of home care experience
• Current Basic Cardiac Life Support (BCLS-HCP)
