This Social Worker serves patients and families admitted to the Trauma program. The work includes psychosocial assessments, counselling, crisis intervention, and linkage to community resources. Team consultation, transitional planning, and interprofessional collaboration are central to the stated duties.

Key Responsibilities:

Provide full-range social work services to Trauma program patients

Complete psychosocial assessments for patients and families

Provide counselling to patients and families

Coordinate discharge planning and transitional planning

Deliver crisis intervention and community resource linkage

Requirements:

Master of Social Work degree required

Ontario College membership in valuable standing required

Minimum two years hospital experience

Trauma experience preferred

Exceptional communication and conflict resolution skills
