The Jean Tweed Centre is seeking a dedicated Case Manager to provide encouragement, emotional support, and practical guidance to clients, families and communities. You will develop holistic service plans, coordinate wrap-around supports, conduct ongoing assessments, referrals, and documentation, while upholding harm reduction and trauma-informed, women-centred practices.

This hybrid role requires a Bachelors in Social Work and at least three years in mental health, substance use, or criminal
