Deliver comprehensive primary care at Somerset West Community Health Centre in Ottawa, using clinical assessment and culturally responsive practice. This 0.7 FTE term role serves diverse patients.

The Family Physician works within SWCHC’s interdisciplinary Primary Health Care Program. You will provide diagnosis, treatment, referrals, health promotion, and disease prevention. Care addresses social determinants of health, continuity, access, and equity for underserved populations.

Key Responsibilities:

- Provide comprehensive primary care across the lifespan
- Assess, diagnose, and manage acute and chronic conditions
- Order investigations and develop evidence-informed care plans
- Prescribe medications and initiate specialist referrals
- Maintain electronic medical records and support follow-up

Requirements:

- Medical degree from a recognized university
- Ontario family physician licence in valuable standing
- College of Family Physicians of Canada certification
- Valid OHIP billing number and current CMPA coverage
- Current CPR/BLS certification and English fluency
