Support adults transitioning home from hospital through occupational therapy, evidence-based interventions, and coordinated community care with VHA Home HealthCare. This position supports UHN@Home.

As an Occupational Therapist, you will assess adults with higher needs and provide consultation, recommendations, and interventions. You will help clients reintegrate into home and community settings through a short-term program lasting up to 90 days. Collaboration with clients and integrated care teams supports safer discharges, greater independence, and coordinated schedules.

Key Responsibilities:

- Assess clients using professional practice guidelines and standards
- Implement interventions based on assessments and client input
- Coordinate care plans and intervention schedules with teams




- Document assessments, treatment plans, progress, and discharges
- Support secure care through continuous quality improvement

Requirements:

- Degree in Occupational Therapy from a recognized school
- Good standing with the College of Occupational Therapists of Ontario
- Valid driver’s license with access to a vehicle
- Compassionate, empathetic, and self-driven approach
- Electronic documentation and virtual care skills
