Lead complex nursing care onsite at LOFT Community Services’ Pine Villa in Toronto, using clinical assessment and interdisciplinary leadership.

This Registered Nurse serves as an independent clinical lead within The Path Home – Pine Villa. The five-month contract supports older adults with complex physical health, mental health, addiction, or cognitive challenges. The RN leads RPNs, supervises PSWs, coordinates delegated acts, and delivers client-centered care.

Key Responsibilities:

- Conduct clinical assessments and develop individualized nursing care plans
- Lead RPN practice and provide direct nursing care
- Assign and monitor delegated activities between RPNs and PSWs
- Oversee medication administration, reconciliation, transcription,



and MAR maintenance
- Collaborate with interdisciplinary teams and community partners

Requirements:

- Positive standing with the College of Nurses of Ontario
- University undergraduate degree in Nursing
- Minimum three years’ home and community-based nursing experience
- Experience supporting older adults with complex challenges
- First Aid and CPR certification required
