ParaMed seeks a Registered Nurse for part time community care across the County Area. You will support patients of all ages with varying care complexity, helping them remain comfortable at home. The work includes patient assessments, care plan monitoring, collaborative rapport building, and professional documentation.

Key Responsibilities:

- Assess individualized patient needs using appropriate tools
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative engagement
- Maintain and update qualified patient records
- Provide required after hours on-call coverage

Requirements:

- Registered Nurse registration with the CNO in good standing
- Valid driver’s license and access to a vehicle
- Willingness to travel between patient residences
- Reliability and flexibility for scheduled visits
- Community care experience is an asset
