Bayshore HealthCare is seeking a compassionate and skilled Registered Practical Nurse/Licensed Practical Nurse to provide one-to-one care for a pediatric client in a home setting.



This is a rewarding opportunity to support a young client transitioning from hospital to home while helping them achieve the highest possible quality of life, comfort, and independence.
The successful candidate will provide family-centered nursing care in accordance with an individualized care plan and work collaboratively with the client, family, and healthcare team to ensure protected and effective care delivery.
As a member of our team, you will:Deliver skilled nursing care to a pediatric client in the home environmentAdminister medications and provide care related to G-tube feedings in accordance with the established care planMonitor and document the client's condition, response to treatment, and any significant changes in health statusAssist with mobility, transfers, and positioning utilizing a Sara Stedy and other prescribed
