As a Licensed Practical Nurse on the Integrated Care Team at Downtown Community Health Center, you will provide client and family-centred care. Work across clinics, sites, services, and outreach or home-based visits. Support people experiencing complex health challenges through primary care, mental health, substance use, and community resources.

Key Responsibilities:

- Assess, plan, implement, evaluate, and document client care
- Provide holistic care across the primary care continuum
- Build therapeutic relationships with clients and families
- Collaborate with interdisciplinary providers and Community Care Services




- Seek direction when client needs are unstable or unpredictable

Requirements:

- Graduate from a recognized Practical Nursing program
- Hold current BCCNM registration as a Licensed Practical Nurse
- One year recent related experience or community clinical placement
- Knowledge of Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care
- Demonstrated interdisciplinary communication, judgment, and computer skills
